Emil Riis scored his fourth goal of the season off a Neto Borges pass, giving the Robins the lead at halftime.
Rangers drew level when Kone took down replacement Harvey Vale’s ball and blasted into the roof of the net.
Northern Ireland international Smyth, another substitute, then headed in Jimmy Dunne’s cross to extend the Hoops’ unbeaten streak to six games.
Both teams came into the game off the back of midweek draws, with the visitors making six changes to their starting lineup, while City manager Gerhard Struber made one change, bringing Mark Sykes in place of Ross McCrorie on the right flank.
Rangers threatened in patches, with half-chances for Koki Saito and Kader Dembele, but City generated the better openings, and Scott Twine should have scored from close range after latching onto a Riis cutback.
However, the Danish forward broke the stalemate when Borges recovered Sykes’ overhit ball on the left wing and, as the QPR defence hesitated, produced a superb cross for Riis to head into the net.
City applied pressure at the opening of the second half, but were unable to extend their lead. Rob Atkinson’s last-ditch sliding block stopped Kone after Saito passed him through on goal.
The R’s fortunes changed after the arrivals of Smyth and Vale, with the latter quickly supplying a cross for Kone, who controlled and fired home the equaliser, his fourth goal since joining the club from Wycombe in August.
The home side appeared to have a better chance of winning, with Anis Mehmeti going close and Atkinson headed a corner just wide of Paul Nardi’s goal.
Smyth, who scored from the halfway line in QPR’s 1-1 draw at Ashton Gate last season, settled the game by nodding Dunne’s ball back across Radek Vitek to give his team their fourth win in six games.

Bristol City head coach Gerhard Struber told BBC Radio Bristol:
“This is a typical moment from the Championship. You have to be sharp in all moments and we have not had the sharpness in two boxes in the second half which you need – in both directions we were not sharp and we gifted this game so cheaply.
“We had enough chances to win this game. They are really good at the moment and you have to stretch your performance because this was not enough.
“It’s a shame we got out with nothing when we can see so many good moments, how we break them down, how we play against the deep block and create chances against them – this was very good to see.
“This is the frustration, I can see my boys in really good moments but in two or three moments we’re not ready and lose this game.”
QPR head coach Julien Stephan told BBC Radio London:
“I’m overjoyed for the supporters and players because they earned this victory. They started the game strongly, and we had several chances in the first 20 to 25 minutes.
After the score, the last 15 minutes [of the first half] were more tough mentally, but there was a strong reaction, and they had a wonderful second half with character, intensity, good energy, and ball quality.” Both the starters and the players that came on in the second half performed admirably.
Defensively, we are able to lessen the threat from our opponents, and the team’s structure is sound. It is not just the defence; it is the entire team. We’re six games unbeaten, and I’m hoping to keep it going after the international break.
When you win two, three games, the confidence increases and you can create positive momentum.”
